Program Specialist, Public Health (Health Homes)

Community Health Plan of Washington (CHPW) has an opportunity for a Program Specialist to support coordination of care for the State’s most vulnerable Medicaid population via the Health Home program. This is a new position, and is a unique opportunity to join a dedicated team and lead the operation and enhancement of CHPW’s current program infrastructure. This role is integral in driving improved quality outcomes and promoting collaboration between CHPW, Lead Organizations, providers and the community.

Job Responsibilities Include:
• Identify and implement clinical and administrative program enhancements Represent CHPW in meetings with Washington state’s HCA (Health Care Authority) and DSHS (Department of Social and Health Services)
• Monitor internal/external reporting requirements, including program status, performance, success narratives and other ad hoc measures in order to reach program milestones
• Achieve compliance with State contract requirements and State TEAMonitor inspections/audits
• Provide technical assistance to Health Home delivery organizations in the development of quality improvement efforts and the implementation of Health Home Services

Qualifications Include:
• Bachelor of Arts degree in Health Administration, Public Health, Public Policy, or related field; or an equivalent combination of education and relevant experience required
• Demonstrated ability to lead programs and projects
