  • Get the Message Out. Partner together with the health plan, local community organizations and the provider community to educate members on the benefits of good eye health and the benefits available in their plan. We have the materials ready to go to for existing member outreach programs and communications. 

  • Member Stratification & Targeting.  Leverage partner data, in conjunction with our data, to identify chronic patients who have not received an annual comprehensive eye exam and/or diabetic retinal exam. Develop a mutually agreeable “chase list” for our team to actively pursue through various communication and engagement strategies.

  • Break Down Barriers.  Coordinate the member visit to the provider through appointment scheduling and reminders. In addition we can partner with the transportation options offered by the local health plan to help break down transportation barriers for senior members.   If members can’t make it to the provider, we have experience deploying retinal imagers in other settings – for example senior fares and primary care offices – to meet members in the community.

  • Ensure the Data becomes Information. Work with the provider community to ensure the appropriate coding is accurate and complete for all members, all while gathering additional information on targeted, chronic population.
