Partnering with Providers to Help Improve Patient Outcomes in Type 2 Diabetes

The prevalence of type 2 diabetes is on the rise — and so is the number of available therapies and interventions. Advances in technology have created new ways to measure and communicate in healthcare, although simply providing these new technologies and tools isn’t adequate. These new advancements become more effective when integrated with existing support systems and used to personalize a patient’s care.

With the support of Verily and Sanofi, Onduo is working towards a better way to manage diabetes by creating a virtual clinic that personalizes patient member care plans and collaborates with providers to extend care, allowing healthcare systems to effectively take advantage of rapidly changing innovations to address individual member needs. Onduo strives to reach the 20% of people who are most complex and cost an average of 12X more than those with well-controlled diabetes - to work towards better outcomes and cost savings.

Onduo: A virtual diabetes clinical with tiered services
By building a virtual clinic with licenses in the continental U.S., Onduo offers multiple levels of care to meet people where they’re at — from coaching support and blood glucose monitors for those with fairly controlled diabetes, to continuous glucose monitoring systems (CGMs) and telemedicine for more poorly controlled or more complex conditions.

RESULTS: Services are offered as clinically needed to members and clients receive solutions that save money at scale. All members receive an app-based behavioral health care program, and additional clinical support is offered to the high-risk population that is driving costs.

 
Partnering with primary care providers for optimized visits and continuous care
Onduo supports primary care providers (PCPs) by providing a snapshot of data and information. And for urgent needs, patients can get video consults with Onduo’s in-house board certified doctors who provide care coordination back to the network.

RESULTS: Provider partners can save 5 minutes from each PCP visit, allowing more conversation and other focus during the visits. Members get access to care beyond the average 30 min/year in doctor visits, to support the 525,600 minutes spent managing diabetes throughout the year.

 

Using data for individual insights and informed, personalized decisions
Members can wear a CGM to see blood glucose readings every 4 minutes — all from a device that sits on the body and doesn’t require active finger pricking*. Members can learn the best foods to eat for their body, what exercises work, and even the effectiveness of their medication.

RESULTS: Of members who reported A1c reduction (Jan 2018 - March 2019), average A1c reduction of 1.2 points. The high risk population (A1c>9) reported the most effect — an average A1c reduction of 3.3.

Access to specialized coaches and nutritionists for improved behavioral and physical health outcomes

With support from endocrinologists, certified diabetes educators, and coaches coupled with connected technology, members receive personalized care based on sophisticated algorithms. Members have access to health care professionals straight from their phone and from the comfort of their home, allowing for hundreds to thousands of touchpoints/year. Members can access diabetes care and tracking tools to ensure they’re getting the right care at the right time to prevent clinical escalations when they need it most — at home, on-demand.

RESULTS: According to Onduo survey data, average Diabetes Distress Scores (DDS) improve by 50.1% and physical activity increases by 41.4%.
 

Leveraging industry leaders including engineers employed by Google
Backed by Verily Life Sciences and Sanofi, Onduo is uniquely positioned to leverage leading science, technology, and diabetes management solutions to benefit patients as well as healthcare partners. A growing number of evidence-based wearables, including Telcare® blood glucose monitors, Dexcom® continuous glucose monitoring system (CGMs), and Orpyx® foot sensor technology, are integrated into Onduo’s care management program to enable data-driven decision making and personalized care.

RESULTS: Of members who signed into the app (January 2019 - August 2019). Majority of members sustain long-term engagement of 70+ weeks with Onduo’s personalized diabetes management program.
