Description of Services: Landmark Health contracts with health plans and health systems in value-based arrangements to manage complex, chronic patients through longitudinal and urgent medical and behavioral health visits. We are a home-based "Complexivist" medical group that does not replace the patient’s PCP but works in partnership with the existing care team. Landmark is a scalable mobile practice, with the ability to treat the patient at home or wherever they reside 24/7/365 and the ability to integrate with the PCP's practice.
