Session description: 

Rich Jones, RN, Director of Medical Management along with Rebecca Tidemann, Population Health Manger, and Jodie Barnett, Disease Management Coordinator from Avera/DAKOTACARE will share an overview of their organization’s evolution from outsourcing Case Management and Disease Management programs to in-house programming to now merging the medical management teams from two separate health plans.  In addition, they will briefly talk about their strategies to integrate population health across the Avera Health System; to unite patients, payors, and providers in a collaborative care design, laying the necessary foundation for a move to value-based reimbursement.  

Please come prepared to share an overview of your DM program and your experiences with external vendors or programs managed in house.  We hope you will join the conversation!
