Care management is evolving at varying speeds but in a common direction throughout the healthcare payer industry. The evolution from a largely telephone-based service to a field-based service, multidisciplinary support system incorporating payee, provider, and community resources, together with patient engagement processes, is a long and difficult path. It is becoming increasingly evident that the transformation is driving toward the eventual integration of public health and private sector medical delivery, blurring distinctions between medical and social support budgets on a health plan, state, and federal level.
