End-stage renal disease (ESRD) patients present many clinical and financial challenges for health plans. New processes and procedures are required to manage quality delivery and control costs for these patients. Health plans gather to share their best practices in handling ESRD patients and discuss their challenges. Come prepared to discuss and learn about new approaches. Some questions to be addressed in the session include:

How are plans handling care coordination for ESRD patients?
How are ESRD patients tracked and reported on?
What lines of business are included?
How are plans coordinating with their providers?
