Manager of Care Coordinator- RN preferred

Health Alliance is seeking a Manager of Care Coordination. The Manager of Care Coordination is responsible for overseeing the daily operations of: case management, social services, utilization team, and associated transitional services. Develops, plans, and manages the functions associated with care facilitationand movement of patients throughout the hospital and in the organization continuum, while promoting effective and efficient utilization of clinical resources. Serves as an expert resource and facilitates changes as needed to keep organization current and compliantwith new regulations, policies and trends related to utilization and care coordination.

EDUCATIONAL REQUIREMENTS
Bachelors Nursing, If not licensed RN, will require MHA, MBA or MSN.

CERTIFICATION & LICENSURE REQUIREMENTS
Registered Nurse (RN) Illinois and National Nursing Certification. National Nursing Certification must be obtained within four (4) years of start date in position. Attend and satisfactorily complete all required continuing education .

EXPERIENCE REQUIREMENTS
Two (2) years of related experience with case management. Experience with care coordination, transitional care planning, population health, discharge planning, clinical documentation and utilization review
