Fly-in: Social Determinants of Health

Event Date: 9/23/2019 - 9/24/2019

Event Overview

Supportive housing is one of the most complicated and expensive social determinants of health.  Join physician leaders from both payer and provider organizations for a deep dive into the unique opportunities we have to positively impact this barrier for the vulnerable populations we serve.  Co-hosted by the Health Plan Alliance and America’s Physician Groups (APG), this executive workshop will offer a unique mix of expert presentations from policy leaders, industry experts; real-world examples from healthcare organizations; and a highly engaging strategic visioning workshop that will allow us to translate ideas into practical applications.

Brought to you by the Health Plan Alliance and America's Physician Groups

The Health Plan Alliance is a national organization that brings provider-sponsored and independently-owned health plans together with their health system and provider group leaders for unparalleled peer-to-peer collaboration. For more than 20 years, Health Plan Alliance members have leveraged the collective knowledge of our community to enhance their business acumen and advance the quality of health care delivery in their communities.

America’s Physician Groups is the nation’s leading professional association for accountable physician groups, composed of over 300 medical groups and independent practice associations (IPAs). America’s Physician Groups’ members operate under a capitated, coordinated care model that is the essence of the nation’s health reform movement from volume to value.

5 reasons you should attend

  1. Hear from Hawaii’s Lt. Governor, Dr. Josh Green, a passionate policymaker and physician out to solve his state’s housing crisis.

  2. Engage directly with APG president Don Crane and Alliance president Joe Lastinger on the current state of innovative partnerships between payers and providers—and what is possible in the future.

  3. Learn from Inland Empire Health Plan and St. Lukes Health System, and how their housing programs have begun making an impact, plus lessons learned along the way.

  4. Participate in a visioning scenario framework to translate ideas into practical application. We will envision what is possible, what is best for the people involved and what is right for healthcare.

  5. Dig into the foundational requirements for health plans and provider organizations to meaningfully address the social needs of the patients we serve.
