Patient Activation is more than engagement. It reflects an individual's current knowledge, skills, and confidence to manage their own health and healthcare. As such, consistent and empirical measurement of patient activation can provide critical insight as to which of your members may be overwhelmed and ill-equipped to follow prescribed treatments or general directions from providers and care management teams.

Measuring patient activation can quickly and proactively indicate individuals and populations that are more likely to utilize ER services and preventable high-cost medical care. Traditional claims and utilization risk models typically miss individuals low in patient activation.

This webinar is designed to help you:

  • Understand the design and science behind the Patient Activation Measure® (PAM®). Learn about the best practice application of PAM in a variety of settings.
  • Discover how patient activation is predictive of an individual's future clinical outcomes, utilization, and cost.
  • Learn how patient activation can guide more personalized, reliable insights to help health coaches and providers support individuals in developing their self-management skills.
  • Understand how using patient activation as a guide and analyzing changes in patient activation over time, resources for member support programs can be more effectively and efficiently allocated.

Health plan case studies will be shared.
