Patient Activation is more than engagement. It reflects an individual's current knowledge, skills, and confidence to manage their own health and healthcare. As such, consistent and empirical measurement of patient activation can provide critical insight as to which of your members may be overwhelmed and ill-equipped to follow prescribed treatments or general directions from providers and care management teams.

Measuring patient activation can quickly and proactively indicate individuals and populations that are more likely to utilize ER services and preventable high-cost medical care. Traditional claims and utilization risk models typically miss individuals low in patient activation.
