Preventing High-Cost Complications in Integrated Care
Event Date: 2/12/2026
Event Overview

11:00 AM – 12:00 PM CT (60 minutes)
Zoom Meeting
Integrated and provider-sponsored health plans often care for people with complex diabetes and multiple comorbidities, placing them at high risk for preventable complications. Diabetic foot ulcers remain one of the most costly and disruptive events within these populations, often leading to avoidable hospitalizations, amputations, and downstream utilization that strain care teams and increase total cost of care. Many times, these complexities are hidden behind hundreds of codes (including general sepsis) and can be difficult to identify.
Join Podimetrics as they share how integrated plans can identify risk earlier, support timely clinical intervention across care teams, and reduce high-cost complications before they escalate. Speakers will share real-world outcomes from health plans and integrated delivery systems to illustrate how virtual care models improved engagement, supported adherence between visits, and reduced unnecessary utilization.
Learning Objectives
- Understand the primary cost drivers associated with diabetic foot complications in integrated and provider-sponsored health plans
- Learn how proactive remote monitoring supports earlier intervention across multidisciplinary care teams
- Review real-world case examples from health plans and integrated systems demonstrating improved outcomes and reduced utilization
- Explore how virtual care models improve patient engagement and adherence between in-person visits
