People with diabetes typically see their PCP for 3-4 30-minute visits each year, but manage diabetes 525,000 minutes each year. Onduo’s Virtual Clinic seeks to provide the necessary support and education to members with Type 2 Diabetes to empower them to make the right choices in daily management of their disease, with the mindset that what happens in primary care stays in primary care. By providing support between visits, Onduo helps prevent unnecessary clinical escalations, helps to prevent progression down the disease continuum, and supports both the PCP and the Health Plan in the quest for improved outcomes.
