Beginning in 2014, Landmark Health launched and evolved a home-based medical management program that is bringing Complexivist® care to local health care communities.  Over 4.5 years, Landmark has scaled this program to 100,000 patients, 13 states, and 24 markets, through focus and innovation in interdisciplinary, physician-led care, 24/7 services in the home, working with PCP networks, and payment models.

Through attending the webinar, participants should be able to: 
Discuss the push for care being delivered to the most vulnerable patients. 
Understand the importance of addressing social and behavioral care issues, in addition to medical needs. 
Discuss the emerging need for a multidisciplinary team to drive longitudinal outcomes for complex patients. 
Identify the operational challenges in deploying an in-home care model. 
Identify key systems and analytics needed to drive positive patient and financial outcomes through risk-based arrangements.
