Beginning in 2014, Landmark Health launched and evolved a home-based medical management program that is bringing Complexivist® care to local health care communities.  Over 4.5 years, Landmark has scaled this program to 100,000 patients, 13 states, and 24 markets, through focus and innovation in interdisciplinary, physician-led care, 24/7 services in the home, working with PCP networks, and payment models.

Through attending the webinar, participants should be able to: 
• Discuss the push for care being delivered to the most vulnerable patients. 
• Understand the importance of addressing social and behavioral care issues, in addition to medical needs. 
• Discuss the emerging need for a multidisciplinary team to drive longitudinal outcomes for complex patients. 
• Identify the operational challenges in deploying an in-home care model. 
• Identify key systems and analytics needed to drive positive patient and financial outcomes through risk-based arrangements.
