Making lifestyle changes is hard. Motiving your members to make lifestyle changes is even harder. But when it comes to employer groups, you have an extra lever at your disposal to motivate and support change. In 2010, Bob Little, Vice President of Underwriting at Capital District Physicians' Health Plan (CDPHP) in New York, stood up CDPHP’s Shared Health program. Over the last 10 years, the program has seen great success with large groups that have more than 50 enrolled that have supported and actively engaged in the program. To date, over 120 large groups have participated in the program.

During this session, Bob will share:

  • What the Shared Health Program provides employers, employees and their brokers
  • Data on renewals and realized savings through variable premiums
  • How the program is achieving lower and less lengthy inpatient hospital visits, less surgical procedures and less ambulatory visits by offering a total population health solution
  • The Shared Health Team structure: employer, broker, CDPHP sales rep, health promotion specialist, case and disease management, pharmacy, behavioral health and quality
  • Employer data reporting
  • Lessons learned
