As plans continue to work with their providers on the implementation of value based payment models and adopt clinically integrated approaches to care delivery, the role of care management in the value based environment is also undergoing changes. During this call, Jane Scott, Senior VP of Professional Services at Gorman Health Group will share an overview of value based models and plan approaches to care management being implemented in successful value based models. She will also offer suggestions for the transition to a more clinically integrated system in addition to approaches being used to measure the ROI for your care management programs in this new environment.
