The COVID-19 pandemic exposed and exacerbated an existing problem: too many Americans deal with insufficient access to quality food. Over 50% of Americans are suffering from at least one chronic disease that can be directly linked to poor nutrition, while people with obesity were up to 92% more likely to die from COVID – demonstrating how vulnerable we’ve become due to poor nutrition. DSNP members are even more likely to be food insecure and struggle with chronic conditions, and plans are looking for guidance on incorporating these solutions into new and existing care models.
 
Leaders from Foodsmart, the largest Telenutrition provider, discuss how comprehensive telehealth and digital nutrition programs fit into a plan’s Model of Care and the impact these programs can have on Quality Metrics, including metrics related to chronic condition reversal/improvement and food insecurity. 

Attendees have the opportunity to learn how these solutions can serve as the foundation for new DSNP care management programs or provide a significant lift to existing care management programs, including how the programs integrate with care teams. Finally, Foodsmart leaders discuss best practices on program adoption to leverage food as medicine as a member growth tactic for new DSNP programs or retention tactic for existing ones.
