Medication non-adherence has been estimated to result in $200 to $300 billion in avoidable medical costs annually. It has also been estimated that 30 to 70% of hospital admissions are directly or in-directly related to medication non-adherence, and nearly 4 million adverse drug events (ADE) result in some type of healthcare intervention each year.

Recognizing the link between improved member medication adherence and reduction of the health plan’s total medical spend, as well as improved HEDIS and STAR performance, HAP Midwest’s leadership team developed a focused program on improving member medication adherence and quality outcomes.

Through a partnership with ExactCare, HAP Midwest developed and launched a customized comprehensive medication management and pharmacy program targeting intervention on its most non-adherent, highest cost dual eligible members.
